This membership agreement is between S.Thetics Medspa and (The name on form below) shall be effective (The Day you submit form below).  The membership is $149.00 a month, which is your “banked balance” to use as you would like, at your own pace. Monthly payments for this membership shall occur on the 15th day of each month in the amount of $149 according to the membership terms 

Automatic Payment Agreement: 

• Paid Month-to-Month $149 with a recurring charge. The banked balance is given to each customer in the form of a gift card in their S.Thetics account.

• Monthly membership payments shall be made in advance by direct debit from the member’s designated credit/debit account. This information will be stored electronically by S.Thetics Medspa. 

• You are responsible to ensure that the credit card number that’s electronically stored with S.Thetics Medspa is valid so that the monthly fee can be charged to that card. You may change the stored credit card at any time, by calling the office. If there is not a valid credit card available with S.Thetics Medspa at the time the monthly fee is charged to that card one must be provided within ten days after the regularly scheduled date for payment. If payment is not received within 10 days of the initial attempt to charge the card on file, S.Thetics Medspa may exercise any and all rights available to it whether, under this agreement or under applicable law, which may include, but are not limited to, suspension of your membership until such payment is made, assessing a late payment fee of $25.00, and/or terminating this agreement.  

• S.Thetics Medspa reserves the right to review memberships periodically. Members will be given 30 days notice in writing of any changes, which include: any increase in membership fees, change in date of automatic withdrawal.

  Membership Term:

• The initial membership period shall be for a period of six months (the “Initial Period”). A  member shall not be entitled to terminate or suspend his/her membership during the initial period. If the contract is terminated before the expiration date of the initial period, there is an early termination fee of $100.00 that will be charged to the card on file.  Any contract that has been terminated prior to the expiration date will have a waiting period of 1 year before being able to reinstate. After the initial period, the membership shall automatically renew monthly on the 15th of each month. Either party may cancel the membership at any time by giving a 30 day written notice to the other party after the initial period. If you cancel your membership and reactivate, after the initial period, you are agreeing to another “Initial period” of 6 months. 

• This Agreement is personal to the member(s) and may not be assigned, transferred or  otherwise disposed of by the member(s). 

• Applicants for membership must be at least 18 years old. Applicants under the age of 18 years old must have parental consent.

  Termination of Agreement:

• In accordance with the Terms and  Conditions, if you are unable to utilize the membership due to (a) death (b) disability  (c) illness (d) injury (e) a change in residence that is more than 25 miles from S.Thetics Medspa (f) your being called to active duty by the United States Military and deployed outside of the state of the S.Thetics primary location or if the membership and/or S.Thetics Medspa location fails to open or permanently discontinued operations; to effectively exercise any right of cancellation, you must notify S.Thetics Medspa of your intention to cancel in writing, in person or by certified or registered mail at the address set forth below.  In the event cancellation is due to (i) illness or injury, a doctor’s certification of the same must accompany your notice of cancellation or (ii) your being called to active duty, you or your legally designated representative must include a copy of your official military orders with your notice of cancellation, with such notice being provided within 30 days after your call to active duty.   

  Terms and Conditions:

  1. S.Thetics Medspa reserves the right to carry, add or eliminate any of the particular products and/or services provided from time to time.

  2. S.Thetics Medspa reserves the right to close and modify facility hours with or without notice.

  3. The membership discount may not be combined with any other promotional offers or promotional gift cards. Excludes Medical Weight Loss and Hormone Replacement Therapy.

  4. No refunds will be given for any unused services or gift card balances. Your “banked balance” cannot be transferred, must be used by you and cannot be used to purchase gift cards for other people. S.Thetics Medspa reserves the right to terminate this membership at any time.

  5. S.Thetics Medspa gift cards may not be redeemed to pay any portion of the membership fee. The membership fee cannot be combined with any other promotion and/or discount.
